Patients Know Best are delighted to have been awarded a place on the new Health System Support Framework (HSSF) service Line 1: Self-Supported-Management Measures – providing measures which assist and enhance supported self-management interventions or that can be used to assess and compare the impact of self-management interventions.

The HSSF provides a quick and easy route to access support services from innovative third party suppliers at the leading edge of health and care system reform.

The new service line of the framework focuses particularly on services that can support the move to integrated models of care based on intelligence-led population health management. Supported self-management (SSM) is a core part of the NHS Long Term Plan’s commitment to make personalised care business as usual across the health and care system. Many NHS organisations are already utilising Patients Know Best to personalise care and support patient self management, which is at the heart of our company’s mission.

Patients Know Best is our social mission. We believe we will achieve this mission when every person:

• Owns a copy of all their health information
• Understands what this information means
• Uses this understanding to make shared decisions with family members or carers and healthcare professionals.

As the company was founded and underpinned by the belief that patient access and ownership of health records can unlock the potential to greater health outcomes and experiences, our aim is for every person to have the ability to access their personal health record, control who can see it and use this information to manage their health and care.
